Covid-19 Vaccination Programme

WE ARE CURRNTLY RUNNING COVID VACCINATIONS CLINICS FOR PATIENTS DUE THEIR SECOND DOSE ONLY.

  • AGED 70 YEARS OR OLDER
  • AGED 65 - 69 AT 31/03/21
  • CLINICALLY EXTREMELY VULNERABLE age 16+ and have a shielding letter
  • AGED 60 - 64 AT 31/03/21
  • AGED 55- 59 AT 31/03/21
  • AGED 50 - 54 AT 31/03/21
  • AGED 16 - 17 YRS at 31/03/21 and received a GP letter
  • AGED 45 - 50 AT 31/03/21
  • AGED 40 - 44 AT 31/03/21
  • AGED 35 - 39 at 31/03/21
  • AGED 30 - 34 AT 31/03/21
  • AGED 25 - 29 AT 31/03/21

Practice Operations

Due to the situation with (COVID-19) and in order to protect patients and staff, we have changed the way we operate.

If you need medical advice please contact the practice any morning before  9 AM and a GP will return your call.  They will assess if you require an appointment at the practice.  If you are asked to attend the practice please wear a face covering.

Treatment Room are offering longer appointment times to allow social distancing - if you require an appointment please contact reception.

Press the buzzer system located at our front doors on arrival and reception will attend to you as soon as possible.  Staff are extemely busy with telephone enquiries but are aware that you have arrived once you buzz, please be patient with us.

Do NOT attend the practice if you are feeling unwell, have a new persistent cough and/or fever, loss of taste or loss of smell.  Return home and self-isolate as per PHA guidelines.  It is not necessary to speak to the doctor unless your symptoms are not manageable at home or do not get better after 7 days.

Prescription Requests

Patients can continue to order scripts in writing by:

1.   Leave your request in the box outside building (24/7)

2.   Email Reception on reception.Z00275@gp.hscni.net - stating Name, Address, DOB  and Names of Meds and Dosage

3.  order online with Patient Access - you will require a PIN for this - this is only available if you already have a pin.  Photographic ID required and must be 18 yrs and older.

Phones

Please do not ring to check your script is ready.   You should allow 48 hours from ordering before collecting from your chosen Chemist. 

Scripts Collection

All scripts must now be collected by a nominated Pharmacy.  If you have not nominated a Pharmacy - please ring the practice to give verbal consent.  You must follow this up with a signed Pharmacy Nomination Form.  Forms can be downloaded below or from outside our Building above our Script Deposit Box.  Please return the completed forms in our Prescription Box.
