New Patient Registration

Park Practice has 4 GP partners.  Once registered you can speak to or see any of the partners not just the GP you are sepcifically registered with.

If you live within the practice area of 5 miles BT23 of the surgery and would like to become a patient you first need to complete two forms.

Please note once you have all the paperwork completed we request you bring it in person to reception so we can verify your ID.

Changing Address

For patients who are already registered and who are changing address, we ask you to complete a change of address form. This form is available at reception or online here.

Please ensure every field is completed and that the names of all the people who are changing address are clearly stated. We need to see proof of address before we can change your address details and we will also need to check that your new address is within the practice area before we can accept your continuing registration at that address.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
